End of life pathway for ages 65+
Home Nursing, Palliative & End of Life Care support in the Illawarra with a specialised and local team. Our team consists of registered nurses, endorsed enrolled nurses and care worlers, all available 24/7 to accommodate the needs of each client and their supporting families.

What we offer:
Sisternightingale is an all-inclusive Nursing service, offering Palliative and End of Life care, Home Nursing & Aged Care services to the community. Sisternightingale is also able to offer all Support at Home services and also provides Clinical Nursing via the End-of-Life Pathway for those aged 65+, which offers $25,000 of funding to be used for Clinical Nursing over a period of up to 4 months. After hours package
Peace of Mind for you and your family after-hours
Designed to assist you and your family during the overnight period when our local care services are not operating as usual. Please speak with us about options for friends and family to contribute to the cost to make this more affordable for you.
Each package includes:

1hr face to face consultation upon commencement of package to establish specific needs for you and your family

24hr phone support (via text or phone call)

Liaison with health professionals eg. GP, Community Palliative Care, Allied health on behalf of clients if reqiured

A more in-depth package for clients with diverse support needs
This will be customised, and will include the after-hours package, plus individualised supports based on client and carer's needs established in the initial free consultation.
