What is the My Aged Care End of Life Pathway?
At Sisternightingale Home Nursing, we understand that the final stage of life is a deeply personal and often overwhelming time — for both the individual and the people who love them. The End-of-Life Pathway, through My Aged Care, is designed to help make this time a little easier. It provides urgent access to additional support at home, so care can be put in place quickly, without the usual waiting periods.
This means that when time matters most, you and your family can focus on being together — rather than navigating services. Through this pathway, increased support can be arranged to meet changing needs. This may include nursing care, personal support, equipment, and coordination of services — all tailored to ensure comfort, dignity, and a sense of calm. At Sisternightingale, we gently guide this process for you.
As a small, locally based team, we take the time to understand what matters most — whether that is remaining at home, being surrounded by familiar people, or simply feeling safe and supported. We work closely with your GP, specialists, and palliative care teams to ensure care is seamless and responsive as needs change. Our role is not only to provide care, but to bring a steady, reassuring presence into the home — supporting both the individual and their family with understanding, respect, and compassion. We are here to walk beside you, helping to create a space where comfort, connection, and dignity remain at the centre of care.
What does it provide?

Up to $25,000 in funding over 12 weeks

Can be extended to up to 16 weeks if funds remain

Who is eligible?
To access the pathway, a person must:

Be eligible for aged care (65+ or 50+ for Aboriginal and Torres Strait Islander people)

Have a doctor or nurse practitioner confirm:

Life expectancy of approximately 3 months or less

Significant functional decline (e.g. mostly bed-bound)
